Waivers
Please read each waiver carefully before signing. Both documents apply to all activities held by Frederick Footy LLC.
Frederick Footy LLC — Participation Waiver
This waiver applies to all activities held by Frederick Footy LLC, including but not limited to practices, pickups, leagues, games, training sessions, clinics, camps, parties, and any other programs or events.
1. Assumption of Risk
I understand and acknowledge that participation in soccer, sports, and recreational activities involves the risk of injury, illness, property damage, or death. These risks include collisions, falls, physical contact, equipment failure, the condition of the playing surface, and the actions of other participants. I voluntarily assume all such risks, whether known or unknown, and whether caused by Frederick Footy LLC or by others.
2. Communicable Disease Acknowledgment
I understand there is a risk of exposure to viruses, bacteria, and communicable diseases, including but not limited to flu, COVID, staph infections, and skin conditions related to sports environments. I voluntarily assume all such risks associated with participation.
3. Turf and Equipment Acknowledgment
I understand that artificial turf, soccer goals, training tools, and other facility equipment may present risks, and I accept full responsibility for participating on or around these surfaces and items.
4. Medical Authorization
In the event of an injury or emergency, I authorize Frederick Footy staff, coaches, volunteers, or facility personnel to secure medical treatment for me or my child. I accept full financial responsibility for all medical services provided. Frederick Footy does not assume responsibility for medical care or related expenses.
5. Release and Waiver of Liability
In consideration of being allowed to participate, I release Frederick Footy LLC, its owners, employees, contractors, volunteers, and affiliated facilities from any and all claims related to injury, illness, property damage, or death that may occur as a result of participation. This release applies to claims based on negligence, premises conditions, or any other legal theory.
6. Indemnification and Hold Harmless
I agree to indemnify and hold harmless Frederick Footy LLC, its owners, employees, contractors, and volunteers against any claims, damages, or costs, including attorney fees, arising from my or my child's participation or actions, including claims brought by third parties.
7. Personal Belongings Disclaimer
I understand that Frederick Footy is not responsible for lost, stolen, or damaged personal items brought into the facility and that I am responsible for securing my own belongings.
8. Behavior and Conduct
I understand that Frederick Footy may remove any participant who violates facility rules, behaves in a disruptive or unsafe manner, or endangers the safety or enjoyment of others. Removal is at the sole discretion of Frederick Footy and without refund.
9. Severability and Governing Law
If any part of this waiver is found unenforceable, the remaining sections will remain valid. This agreement is governed by the laws of the State of Maryland. Any legal claims must be brought in the appropriate Maryland courts.
10. Acknowledgment and Signature
I have read and understand this agreement. I voluntarily accept all terms. I understand that by signing this document I am waiving certain legal rights.
